WORKUP

If fever is present, consider searching for the following infections: 

Herpes Simplex Virus (HSV) 1 and 2

  • HSV 1 IgM and HSV 1 IgG

  • HSV 2 IgM and HSV 2 IgG


Mycoplasma Pneumoniae

  • Chest x-ray

  • Mycoplamsa polymerase chain reaction (PCR)

  • Direct Coombs test

  • Cold-agglutinin


Histoplasmosis

  • Ask about exposure to Ohio and Mississippi River Valleys

  • Fungal cultures from bronchoalveolar lavage (BAL)

  • Antigen detection enzyme immunoassay (EIA) in urine, blood or BAL fluid

  • Serologic testing

Orf

  • Ask about exposure to sheep or goats

  • Biopsy or molecular testing of skin lesion

  • Look for erythematous nodule on the hand
